Basic Info for previews and reviews - Whichever you're doing, start with this info!
Kit information
Kit description (e.g., Z-models 1/72 FartKnocker)
Kit stock number
Price, and where you got it
Accessory information - aftermarket options/modifications, other notes specific to your review
Specific info for previews - Add this to the basic info above for a preview!
Subject history - one or two paragraphs, or if you're lazy a link to a good site
Kit decription - Your observations on the kit, composition, etc.. Include pics of sprues, decals, etc
References, if any used

Subject history & kit description
Contruction - as descriptive as possible
The intent is to help someone else decide if they can handle this kit, or how they might go about building it, avoiding kit-inherent problems, etc. Try to include oft-forgotten in-progress photos of the kit as you build it.
Painting/Decaling
Include info on your scheme research, what colors you used, how they went on. Same for the decals.
Conclusions
Overall impressions of the model and project. How satisfied are you with the result of your kit/project? Was it hard/easy/PITA? Final parting shots, recommendations, etc.
References - Any/all sources you used to help you build this kit. Include website URLs if applicable
Final general guidance - As a rule, it's best to sit on your preview/review for a day. I can't tell you how many mistakes I've found after "sleeping on it" and looking at it after a short break. Please check your review for good logical flow and spelling. I can help tweak it, but I don't want to be in a position of having to re-write your review. Also, do not feel compelled to write a book, and so put off doing a review at all. As long as you adequately describe what you've done for the readers, there's no need to kill yourself.
